
The Jackson R-2 Health and Wellness Committee will be promoting the "Live Healthy" programs for the EIGHTH year. During the 2018-19 school year, each employee will have an opportunity to participate in a variety of wellness activities and earn points for their participation. At the end of the school year, gift cards will be awarded.
Reward point totals/gift card amount: 200+ points will earn $100 gift card, 140-199 points will earn $50 gift card and 95-139 points will earn $25 gift card.
Employees that participate will have a Live Healthy book that will need to be initialed after each activity is completed by a school nurse, dept. supervisor, physician or nurse at physician office. The Administration office, Support Services, and each school nurse has a supply of books.
Approved Health Activities/points allowed: Beginning of year screening: 50 points (online Health Risk Assessment ONLY=20 points, Profile and lab work (either at screening or at your physician's office )=30 points. Still need to attend screening, but you may bring a copy of your most recent labs (within 3 months). Instructions to complete the online Health Risk Assessment will be emailed out by Ellen Gipson.
Live Preventative: 5 points
Flu vaccination, Annual physical exam, Dental check up and cleaning, Annual OB/GYN, Mammogram, Physical Prostate exam, Blood/Platelet Donation, Vision Examination, Colonoscopy: 5 points each (can only count one time/school year). Medical appointments made during June and July can be applied to the upcoming school year's wellness points.
Step it Up Challenge, Any Local 5k (or longer) race, All-levels Yoga (Fall & Spring sessions), Maintain Don't Gain, Personal Live Healthy Challenge, WOW6 Challenge, Shape Up My School, Couch to 5K Training program, and Jackson R-2 Milk Mustache Dash: 15 points each.
Personal Live Healthy Challenge: 15 points if participate, 30 points if successful with challenge
Live Healthy Talks: 15 points each (total of 6 talks); Free child care provided! Times and days of the week vary.

The completed Live Healthy Booklets (with points added up) can be turned in to your building nurse by May 6th . Any gift cards earned will be distributed to your building in May.
