The Jackson R2 Health and Wellness Committee is initiating a new program for the 2011-2012 school year called "Live Healthy". During the school year, each employee will have an opportunity to participate in a variety of wellness activities and earn points for their participation. At the end of the school year, gift cards will be awarded.
Reward points: 200+ points will earn $100 gift card, 140-199 points will earn $50 gift card and 95-139 points will earn $25 gift card.
Employees that participate will have a Personal Profile book that will need to be initialed after each activity is completed by: school nurse, dept. supervisor, physician or nurse at physician office, or presenter of talks. Each school nurse has a supply of books.
Approved Health Activities/points allowed:
Beginning of year screening: 50 points
Smoking Cessation program: 30 points
Influenza Vaccination: 15 points
Annual physical exam, Dental check up and cleaning, Pap smear, Mammogram, PSA blood test, Prostate exam, Chest Xray, Colonoscopy: 5 points each (can only count one time)
Fall Shape Up, Maintain Don't Gain, Biggest Loser, Walk Across America, End of Year Run/Walk: each 15 points
Talks: Exercise Prescription, Nutrition and Supplementation for the Busy Life, Money Management, Self Maintenance 101, Be Nice to Your Feet, Cancer Facts: 15 points each
