Absence or Travel Request
Step 1 The Absence Travel Request (updated 8/22/08). You may also obtain this form from the school secretary. The top half of the form should be completed by the teacher.
a. A separate form should be completed for each trip.
b. A separate form should be completed for each participant.
c. If registration is required, a completed form should be attached.
d. If lodging is required, individual attending the meeting is responsible for making reservations. Please contact hotel for reservations, total room charge, confirmation number (write confirmation number on form), and ask if the hotel accepts Purchase Orders. Inform hotel that Jackson R-2 School District is tax exempt . Please note that certain local taxes may still apply and these taxes need to be included in the total room charge.
Step 2 The Absence/Travel Request and Reimbursement form is then turned into the office for the principal's approval and fund account number.
Step 3 Following principal's approval, the travel request form is sent to the board office for approval by Dr. Fisher. The Board office should receive forms a minimum of two weeks prior to registration deadline.
Step 4 Dr. Fisher then gives the form to Mary Bollinger at the Board Office for purchase orders to be completed.
Step 5 The Absence/Travel form and Purchase Orders are logged and processed.
Step 6 The teacher receives the Absence/Travel Request and Reimbursement form back, along with copies of the lodging and workshop registration Purchase Order.
Step 7 The reimbursement section is completed following the activity. Return the completed form to Mary Campbell, Accounts Payable for check processing. All receipts/invoices must be attached.
Remember to turn in Absence/Travel Request form for reimbursement by Wednesday the week prior to the board meeting.
Board meetings are the 2nd and 4th Tuesday of each month except July and December when there is only one on the 2nd Tuesday
